Accommodation and support for adults who have/are experiencing issues with mental health
We’re here to help
The service will offer support to enhance or develop skills to maintain a tenancy; educate in relation to your mental health, to engage with various coping strategies understanding your medication regime.
Staff are available 24 hours, we will provide support with handling finances, daily living skills such as shopping, laundry, hygiene. Support will be offered with engagement in meaningful activities such as education, voluntary work, work placements or paid employment. Emotional support, 1:1 support is tailored to suit your needs. You will be involved in all aspects of your care and support.
You will be allocated a specific Key Worker; however, we operate a full team approach to ensure you are fully supported. 3 monthly Review Meetings are held with yourself, and your external care team to assess the care and support provided is relevant.
Staff will support with all aspect of identifying your move on accommodation.
Who is this service for
- People aged 18 or over
- People living in or have a family connection to the Redcar & Cleveland area
- People under the care of a local community mental health team
Get support
All referrals for the service must go through a Social Worker, who then presents your details to an Accommodation Plan, then decides which service would be most suitable to support you.
