We’re here to help

Chadwick Fields is a mental health floating support service based in Middlewich, with 12 individual self-contained flats. We provide a person-centred holistic recovery focused approach and promote the recovery of the individuals who live here and support them to regain their independence. 

You will be designated a key recovery worker who you will be required to meet with your key worker once a week (or however many appointments you have been allocated) to have a key working session.  

These sessions will allow you to discuss anything you wish to discuss or raise any issues you may have. During your initial support session, you will be supported and encouraged to develop your own person-centred support plan, using SMART (Specific, Measurable, Attainable, Realistic and Timed) goals. Next support sessions will be used to work towards these. 

We can support with: 

  • Signing a Muir tenancy within the building.  
  • Setting up utility bills, gas, water, electric, TV, council tax 
  • Maximise benefits, apply for any eligible exemptions, discounts or grants 
  • Identify, apply and attend any course to enhance basic skills needs, employment, wellbeing needs 
  • Build positive relationships and reconnect at service level within the community and where requested and appropriate family and friends 
  • Identify, apply and attend employment opportunities paid or voluntary 
  • Maximise health and wellbeing, book and attend any appointments such as GP, dentist, CMHT, order, store and oversee medication 
  • Organise and facilitate activities such as walking groups, cinema groups, activity days, service user meals 
  • Register for local housing authority/private property owners and actively bid whilst gaining the skills to live independently
