Horsham Pathfinder
If you are 18+, living in the Horsham District area and experiencing challenges with your mental health, we can help to improve your emotional wellbeing with guidance, one-to-one and group support.
We’re here to help
We provide short term support for those 18+ living in the Horsham District area, who need support with their mental health. We provide up to 12 one-to-one support sessions with a Peer or Recovery Worker and run a range of groups, in 12-week cycles, that we regularly review and update. We mostly provide practical support for those struggling with depression, anxiety, stress, and confidence & self-esteem. We are always open to discussing any other mental health needs too, and how we can provide support where needed. This support mostly consists of coping strategies, wellness plans, and other management techniques to help you manage and cope with everyday living and your emotional wellbeing.
Our referral process normally begins with our referral form, and from there our Referral Coordinator will be in contact to discuss your support needs, and potentially book you in for an initial discussion appointment. This is an informal assessment that helps us understand how you have been feeling, your current situation, and helps us build your support plan. From this appointment, you will be allocated a Peer or Recovery Worker, who will be your support during your 12 sessions.
We also provide further signposting advice and guidance for those that may have any further support needs, e.g., finances, employment, physical health needs, etc. We are always open and happy to have discussions about the issues you may be facing and the most beneficial pathway for you to access the support you need. We also work alongside many other local services, to ensure that people are being supported in all areas.
The Emotional Wellbeing Service works out of 4 GP surgeries in Horsham. Park, Orchard, Riverside and Holbrook. The EWS offers 6 one-to-one support sessions with a mental health support coordinator, which can be offered in person, over the phone or virtually.
The MHSC’s provide practical support for those struggling with depression, anxiety, stress, low confidence and self-esteem. We are always open to discussing any other mental health needs too, and how we can provide support where needed.
